This site is informational. It does not collect personal health information, and there are no forms on it that transmit your details to us. Booking and referral both happen by fax and telephone.
Personal health information
Vaughan Endoscopy Clinic collects, uses and discloses personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). Information is collected for the purpose of providing care, and is shared with your referring physician and, where relevant, with other members of your circle of care.
Your rights
Under PHIPA you have the right to access your own health records, to request a correction where you believe information is inaccurate, and to raise a concern about how your information has been handled.
