For referring physicians
Fax the referral form and medical history form to 905-856-2602. We contact your patient directly to book, send their preparation instructions, and report back to you within 24 hours of the procedure.
The three steps
-
Check the patient meets our health criteria
We are an out-of-hospital premise. Patients who fall outside the criteria below are safer having their procedure in a hospital setting, and should be referred to one of our gastroenterologists’ offices for consultation instead.
-
Fax the completed forms to 905-856-2602
Both the referral form and the medical history form. We cannot book without the history.
-
We take it from there
Once both forms arrive, we contact the patient directly to arrange a convenient appointment, and provide them with bowel preparation instructions and informed consent documentation.
Please do not refer to VEC if the patient
These exclusions exist because we are an out-of-hospital facility without the escalation capacity of a hospital endoscopy unit.
Cardiac and respiratory
- Has significant cardiovascular or respiratory disease
- Had a myocardial infarction within the past year, or has cardiac stents
- Requires prophylactic antibiotics for previous endocarditis, a mechanical heart valve, or complex congenital cardiac abnormality
- Uses CPAP for sleep apnoea and has a BMI over 35
Other exclusions
- Has significant or severe gastrointestinal complaints
- Has significant renal, neurological or liver disease
- Is an insulin-dependent or brittle diabetic
- Is on Coumadin, Plavix, subcutaneous heparin, Ticlid, Pradaxa or other anticoagulants
- Is on continuous narcotic therapy
- Is morbidly obese, BMI over 40
- Is under 18 or over 80 years of age
Higher-risk patients still have a route
A patient who does not meet the criteria is not a patient we cannot help. Refer them to one of our gastroenterologists’ offices for consultation, and the appropriate setting for their procedure can be arranged from there.
What you get back
- A report faxed to your office within 24 hours of the procedure.
- Follow-up arranged by the endoscopist, who will look after your patient for any endoscopic findings.
- Histology results once the laboratory has reported, where biopsies or polypectomy were performed.
Combined procedures
If your patient is booked for a colonoscopy and has symptoms warranting an upper endoscopy, a gastroscopy can be pre-arranged for the same visit. Note it on the referral form.
